It seems that your console is having problems reading the hard drive ("HDD") or Blu-ray / DVD Drive, please follow these steps:
Step 1: Remove the disc from the console, making sure it's not dirty, scratched or damaged.
Step 2: If the drive is in good condition please try to rebuild the database system using Safe Mode 5. Information on how to access and use the safe mode option can be found here.
